Found out the hard drive needed to be cleaned out by Rebuilding Database in Safe Mode

It seemed nothing was wrong after rebooting the system, but as time went on it got slower, data was becoming corrupted, system errors advising to rebuild the database in safe mode, until the hard drive finally failed completely.

Replace the hard drive as stated for your model PS.Turn your PS4 on in safe mode – like the directions, press the power button and hold, it will beep once, then beep again
